FAVN - SUBMITTING CLINICS ONLY- RESULT SENd-OUT PREFERENCE

 

It is the responsibility of the submitting clinic* to ensure their name, postal address and the email or fax (required for receiving results), is correctly entered on the test submission forms for FAVN and RFFIT. This information is entered into an account, and all future samples received in the laboratory will be match to the clinic’s account via the clinic’s name and postal address on the submission form. If the name of the submitting clinic is not an exact match between the submission form and the clinic name as it appears in their account, delays to receiving results may occur or the sample may be matched to the wrong account. If a sample is matched to a wrong account, it can lead to billing and result send-out errors. A copy of the results for FAVN and RFFIT tests are sent via email or fax to the submitting clinic. For international clinics we recommend email for receiving the copy of test reports.

Specific to FAVN tests: Do not include anyone’s contact information on the FAVN REPORT FORM that is not employed at the submitting clinic (such as a pet owner), as it will be entered into the clinic’s account for all future result send-outs. Please see Sendout of FAVN REPORT FORM in the FAVN test instructions for how to request that a result be sent directly to a pet owner. The original FAVN report (hardcopy stickered document) will be mailed via USPS (United States Postal Service)** to the submitting clinic, unless we have written permission from the submitting clinic to send it elsewhere, or are required to send it to a quarantine station such as Hawaii, Guam or an island of the Caribbean***. If the original FAVN report goes to a quarantine station, a copy ONLY is sent to the submitting clinic via email or fax. A copy of results is always sent to the submitting clinic, even when an original FAVN report is sent.

If an original FAVN report never arrives, and it is required for travel, there will be a fee to reissue a new original FAVN report. If a postal address written on the FAVN REPORT FORM does not correspond exactly to the address we have in the clinic’s account, the address in the account may be changed to match the postal address as written on the FAVN REPORT FORM.
It is also the submitting clinic’s responsibility to notify the Rabies Laboratory in writing of any changes to their result send-out information before sending samples. If you have any questions, please contact us at rabies@vet.ksu.edu or 785-532-4483.

* The Rabies Laboratory considers the submitting clinic listed on the FAVN REPORT FORM its client, regardless of who pays for testing and who ships the sample to the laboratory. The pet owner is not the considered the client. Please see Payment Options and Billing Policy for further details.

**When the original FAVN report is mailed via USPS it is not possible to track or predict when the document will arrive. It is highly recommended to request courier delivery (see FAVN test instructions) of the original FAVN report if you require it for travel, especially for clinics located outside the continental United States of America.
