Network Account Request

 

All KSUCVM faculty and staff requesting access to the CVM network are required to fill out and sign a network request form.  The request forms are available for pickup at the following locations:

406 Trotter Hall
9 Trotter Hall
111-A Mosier Hall

The request form is also available on-line by clicking on the following link to open the form in Adobe Acrobat (PDF) format. You may then use the your browser to type in the information.  Then, print the form, sign it, and turn it in to one of the addresses listed on the form.
