Presenting Complaint
Anorexia and vomiting for 5 days. Vomit material is clear, bile colored.
Physical Examination
- Depression and lethargy
- Dehydration
- Tachycardic (160bpm)
- Hypersalivation
- Nasal discharge
- Mydriasis
- Decreased anal tone
- Decreased PLR
- Elevated nictitans
- Prolonged Schimer tear test
- Non-atropine responsive (no increased heart rate)
- Intradermal histamine - mild flare with minimal wheal
- Ophthalmic pilocarpine test - meiosis in 17 minutes
Hematological Findings
- Chloride - 95mmol/L (109-124mmol/L)
- HCT - 55.7% (37-55%)
- Monocytes - 2500/ul (100-800/ul)
