Biopsy of right ischium was surgically done and submitted for culture, cytology, and histopathology.
Culture: Negative
Cytology: Pleomorphic spindle cells were seen that may suggest neoplasia or a reactive process. Osteoblasts, a few small lymphocytes, occasional neutrophils, and rare osteoblasts were also noted.
Histopathology: Chronic osteomyelitis with osteonecrosis, osteolysis, and skeletal muscle myodegeneration and mild myositis.
Treatment: Six-week course of antibiotics.
Future plan: Repeat physical exam and radiographs to note improvement or need for alternative treatment.
